This is a standard template that you can send to clients to gather the information you may need for your party. It is based in a word document and can also be used in google docs. The elements that are highlighted red are for you to complete and the other sections are for the customer. If you are providing personalised items you would need to check the spelling of the guests names. I usually do this 14 days before the party to ensure that the full payment is received before any personalised items are made.
